The purpose of this assistance program is to help defray the burial costs associated with the passing of one of our honored Public Safety Officers.
Applications are accepted on an as needed basis and will be considered immediately upon receipt and formation of a quorum by The Selection Committee.
Applications for assistance will be accepted from any legally designated immediate family member or legally chartered Public Safety Officer Association or Organization acting on behalf of a Fallen Public Safety Officer who becomes deceased while serving on
Active Duty in the State of Florida.
Assistance will be awarded by The Chief Chris Adams Memorial Fund – Family Assistance Program Selection Committee.
Assistance will be awarded based on the total application package information provided.
All information provided will be vetted. It is our intent to verify the information provided as quickly as possible in order to render assistance in a timely manner.
Assistance award amounts will be determined by The Selection Committee and the requester will be notified promptly.
Assistance funds will be paid directly to the Attending Funeral Home and facilitated by the Banking Institution servicing the non-profit, charitable account(s) for The Chief Chris Adams Memorial Fund.
Application for, Awarding of and Acceptance of Assistance from The Chief Chris Adams Memorial Fund - Family Assistance Program does not guarantee all expenses related to burial will be provided for. The Memorial will provide assistance in the greatest amount possible.
The recipient of assistance from The Chief Chris Adams Memorial Fund - Family Assistance Program agrees not to hold The Memorial liable in any circumstance related to The Memorial’s assistance in this or any other associated matter.

Application process can be initiated by telephone to any committee member. The completed application must be received and approved by The Selection Committee before assistance can be awarded.
